There are three ways to register.
     
1. Print form and mail with $250 deposit to: Pittsburgh Indoor Sports Arena  
  220 Rich Hill Road

2. Print form and fax to 412-820-7658 and then call 412-820-0657 for credit card payment
     

3. Click on Send Form and call 412-820-0657 for credit card payment

     
Effective this season, $50.00 of the $250 deposit will be required for a cancellation deposit which is due upon registration.  This is an additional fee not included in league fees.  Any cancellations or no-shows will result in a forfeiture of this deposit.  All deposits will be returned at season's end for qualified teams.
